SERVICES
Transitioning Adults plus provides comprehensive Care Management services for older adults and adults with disabilities. We work closely with the individual and their supportive family members to develop a Life Plan that addresses not only needs but also desires of the individual in transition. The process provides education, resources and access to our trusted professional network for services. The Care Liaison may coordinate the plan and provide ongoing support or assist the family to implement the action steps. Because each case is different the person-centered approach provides results specific to your needs.
Whether you are returning to the workforce following a disabling condition or planning for the next stage of your retirement we offer services to help your family experience Peace of Mind knowing you have the best plan in place for yourself or your loved one.
